Become A Client:

Referrals are received through the Agency for Persons with Disabilities, Polk County schools, Waiver Support Coordinators, and private referrals.  If you do not qualify for Medicaid funding, AFI also accepts private pay. If wait time for services occurs, future AFI participants shall be placed on a waiting list in the order received.

Prior to acceptance in our program, the applicant is required to schedule an initial tour of the agency. Every applicant will then be given an admissions application packet.  The fully completed admissions application packet is reviewed by the Support Services Team for a majority vote for admission to the program.  The Support Services team consists of the Director of Program Operations, Case Manager and Program Specialist(s). Final approval is given by the CEO.

Entrance Criteria:

Applicants interested in the Adult Day Training program must meet the following criteria:

  • Individuals must be 22 years of age or older. Individuals 18 years or older may be considered if their needs can’t be met through the public school system.
  • Individuals must have a Social Security number.
  • Individuals must be medically stable and their seizures controlled to the point that their care does not require medical personnel or pose a threat to the safety of others or themselves.
  • Individuals must be continent and be able to care for their own bathroom needs. They must also be able to feed themselves independently.
  • Individuals must be ambulatory or have adaptive devices for mobility.
  • Individuals with behavior issues are accepted on a case-by-case basis after review by Support Services staff and the President/CEO.
